We are seeking an experienced Social Worker to join our team at Quality Talent Group. As a Social Services Coordinator, you will play a vital role in ensuring the social and emotional needs of our residents are met.

Key Responsibilities:

  • Maintain and improve residents' ability to control their physical and mental/psychosocial needs.
  • Identify and address individual social and emotional needs.
  • Develop and maintain individualized care plans.
  • Participate in the planning of residents' admission, community reentry, or transfer to another facility.
  • Participate in interdisciplinary care plan meetings to ensure treatment of social and emotional needs.

Requirements:

  • Bachelor's Degree in Human Services, Psychology, or Social Work.
  • Long-term care/subacute experience required.

A competitive hourly rate of $35-$40.
