LPN - Primary Care Nurse

Stamford Health, a leading healthcare system in Connecticut, is seeking a skilled Licensed Practical Nurse (LPN) to join our Primary Care team in Darien. As a valued member of our team, you will play a critical role in delivering high-quality patient care and contributing to our mission of providing exceptional healthcare services.
Job SummaryThe LPN will be responsible for coordinating patient care, providing mentorship to non-nursing personnel, and serving as a resource to staff and management. Key responsibilities include:
- Checking schedules and organizing patient flow
- Greeting patients, documenting concerns, and preparing patients for exams
- Obtaining and accurately documenting patient vital signs and reporting abnormalities to physicians
- Assisting with specimen collection and testing, and documenting pertinent information
- Providing minor treatment as directed by physicians and assisting with minor procedures
- Providing education to patients and families, and documenting all patient-related activities
- Scheduling patient testing and pre-certifying insurance requirements
- Responding to telephone messages and end-of-month checks for expired supplies and medication
To be successful in this role, you will need:
- A graduate degree from an accredited school of nursing
- A valid license as a Licensed Practical Nurse in Connecticut
- A minimum of 6 contact hours of continuing education annually
- Excellent customer service and communication skills
- Knowledge of medical terminology
- A minimum of 2 years of experience in a similar position
