LPN - Primary Care Nurse

Stamford Health is seeking a skilled LPN to join our Primary Care team in Stamford. As a Licensed Practical Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare system.
Responsibilities:- Coordinate patient care according to established standards of practice and state and federal guidelines.
- Provide mentorship to non-nursing personnel and serve as a resource to staff and management in problem identification and resolution.
- Check schedules, organize patient flow, and greet patients, documenting their concerns and questions.
- Accompany patients to exam/procedure rooms and prepare them for physical exams.
- Obtain and accurately document patient vital signs, weight, and report any abnormalities to physicians.
- Assist with specimen collection and testing as necessary and document pertinent information.
- Provide minor treatment as directed by physicians and assist with minor procedures.
- Provide education to patients and families as indicated after assessing their learning needs.
- Document all patient-related activities by the end of the shift.
- Schedule patient testing and pre-certify if insurance requires.
- Respond to telephone messages by the end of the shift.
- End-of-month checks for expired supplies and medication, and stock rooms/maintain inventory of exam/procedure rooms.
- Order supplies as needed and clean the exam/procedure room and equipment after each patient.
- Notify patients of test results as per physician instruction and follow up.
- Attend scheduled staff meetings, in-service education opportunities, and participate in continuing education programs.
- Graduate of an accredited school of nursing.
- Valid license as a Licensed Practical Nurse in CT.
