LPN Primary Care Nurse

Stamford Health, a leading healthcare system in Connecticut, is seeking a skilled LPN Primary Care Nurse to join our team. As a Licensed Practical Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.
Job Summary:The LPN Primary Care Nurse will be responsible for coordinating patient care, providing mentorship to non-nursing personnel, and serving as a resource to staff and management. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Responsibilities:- Coordinates patient care, including scheduling, organizing, and preparing patients for exams and procedures.
- Obtains and accurately documents patient vital signs, weight, and reports any abnormalities to physicians.
- Assists with specimen collection and testing as necessary, and documents pertinent information.
- Provides minor treatment as directed by physicians, assists with minor procedures, and educates patients as indicated.
- Documents all patient-related activities by the end of each shift.
- Schedules patient testing and pre-certifies if insurance requires.
- Responds to all telephone messages by the end of each shift.
- End-of-month checks for expired supplies and medication, and stocks rooms/maintains inventory of exam/procedure rooms.
- Cleans the exam/procedure room and equipment after each patient, and prepares the room for the next patient and physician needs.
- Notifies patients of test results, as per physician instruction, and follows up as necessary.
- Attends scheduled staff meetings, in-service education opportunities, and participates in continuing education programs.
- Graduate of an accredited school of nursing.
- Valid license as a Licensed Practical Nurse in Connecticut.
