Job Summary

The Stamford Health system is seeking a skilled Licensed Practical Nurse (LPN) to join our Primary Care team in Stamford. As a LPN, you will play a vital role in coordinating patient care, providing mentorship to non-nursing personnel, and serving as a resource to staff and management.

Key Responsibilities
  • Organize patient schedules, greet patients, and document their concerns and questions.
  • Accompany patients to exam/procedure rooms, prepare them for physical exams, and review patient medical profiles and H&Ps.
  • Obtain and accurately document patient vital signs, weight, and report any abnormalities to physicians.
  • Assist with specimen collection and testing as necessary, and document pertinent information.
  • Provide minor treatment as directed by physicians, assist with minor procedures, and provide education to patients and families as indicated.
  • Document all patient-related activities by the end of each shift, schedule patient testing, and pre-certify if necessary.
  • Respond to telephone messages, check for expired supplies and medication, and maintain inventory of exam/procedure rooms.
  • Prepare exam/procedure rooms for next patients and physicians, and notify patients of test results as per physician instruction.
Requirements
  • Graduate of an accredited school of nursing.
  • Valid license as a Licensed Practical Nurse in CT.
  • Maintain a minimum of 6 contact hours annually.
  • Excellent customer service and communication skills.
  • Knowledge of medical terminology.
  • Minimum of 2 years of previous experience in a similar position.
