LPN - Primary Care Nurse

Stamford Health is seeking a skilled Licensed Practical Nurse (LPN) to join our Primary Care team in Darien. As a valued member of our healthcare team, you will provide high-quality patient care and support our mission to deliver exceptional healthcare services.
Responsibilities- Coordinate patient care and provide mentorship to non-nursing personnel.
- Assist with patient interviews, vital sign collection, and specimen collection.
- Provide minor treatments and assist physicians with procedures.
- Document patient information and maintain accurate records.
- Schedule patient testing and pre-certify insurance requirements.
- Respond to patient and physician office messages.
- Maintain exam rooms and equipment, and order supplies as needed.
- Graduate of an accredited school of nursing.
- Valid license as a Licensed Practical Nurse in CT.
- Minimum of 2 years of experience in a similar position.
- Excellent customer service and communication skills.
- Knowledge of medical terminology.
