LPN Primary Care Nurse

Job Summary:The LPN Primary Care Nurse will be responsible for coordinating patient care, providing mentorship to non-nursing personnel, and serving as a resource to staff and management. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities:- Coordinates patient care, including scheduling appointments, obtaining vital signs, and documenting patient information.
- Provides mentorship to non-nursing personnel, including training and guidance on patient care procedures.
- Serves as a resource to staff and management, providing guidance and support on patient care and operational issues.
- Assists with specimen collection and testing, as needed.
- Documents all patient-related activities, including patient interactions, test results, and other relevant information.
- Schedules patient testing and pre-certifies insurance requirements, as needed.
- Responds to all telephone messages, including patient and physician office calls, by the end of each shift.
- End-of-month checks for expired supplies and medication, and stocks rooms/maintains inventory of exam/procedure rooms.
- Cleans the exam/procedure room and equipment after each patient, and prepares the room for the next patient.
- Notifies patients of test results, as per physician instruction, and follows up as needed.
- Attends scheduled staff meetings, in-service education opportunities, and participates in continuing education programs.
- Graduate of an accredited school of nursing.
- Valid license as a Licensed Practical Nurse in Connecticut.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Ability to provide mentorship and guidance to non-nursing personnel.
