LPN Primary Care Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Primary Care team at Stamford Health. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare system.
Responsibilities:- Coordinate patient care according to established standards of practice and state and federal guidelines.
- Provide mentorship to non-nursing personnel and serve as a resource to staff and management in problem identification and resolution.
- Check schedules and organize patient flow, greet patients, document patient concerns, and accompany patients to exam/procedure rooms.
- Obtain and accurately document patient vital signs, weight, and report any abnormalities to physicians.
- Assist with specimen collection and testing as necessary and document pertinent information.
- Provide minor treatment as directed by physicians and assist with minor procedures.
- Provide education as indicated after assessing the learning needs of patients and their families.
- Document all patient-related activities by the end of the shift.
- Schedule patient testing and pre-certify if insurance requires.
- Respond to all telephone messages by the end of the shift.
- End of month checks for expired supplies and medication, stocks rooms, maintains inventory, and orders supplies as needed.
- Cleans the exam/procedure room and equipment after each patient.
- Prepares exam/procedure rooms for next patients and physician needs.
- Notifies patients of test results per physician instruction and follow-up.
- Attends scheduled staff meetings, in-service education opportunities, and participates in continuing education programs.
- Graduate of an accredited school of nursing.
- Valid license as a Licensed Practical Nurse in CT.
