LPN - Primary Care Nurse

The Stamford Health system is seeking a skilled Licensed Practical Nurse (LPN) to join our Primary Care team in Stamford. As a key member of our healthcare team, you will provide high-quality patient care, mentor non-nursing personnel, and serve as a resource to staff and management.
Key Responsibilities- Coordinate patient care, adhering to established standards of practice and state and federal guidelines.
- Provide mentorship and guidance to non-nursing personnel.
- Assist patients with daily activities, such as walking, transferring, and dressing.
- Obtain and document patient vital signs, weight, and report any abnormalities to physicians.
- Assist with specimen collection and testing as necessary.
- Provide minor treatment as directed by physicians and assist with minor procedures.
- Document all patient-related activities by the end of each shift.
- Schedule patient testing and pre-certify if required by insurance.
- Respond to telephone messages by the end of each shift.
- End-of-month checks for expired supplies and medication.
- Stock rooms and maintain inventory of exam and procedure rooms.
- Order supplies as needed.
- Clean exam and procedure rooms and equipment after each patient.
- Prepare exam and procedure rooms for the next patient and physician needs.
- Notify patients of test results, as per physician instruction, and follow up.
- Attend scheduled staff meetings, in-service education opportunities, and participate in continuing education programs.
- Formulate and maintain effective working relationships with employees and other managers of the healthcare team.
- Graduate of an accredited school of nursing.
- Valid license as a Licensed Practical Nurse in CT.
- Maintain a minimum of 6 contact hours, annually.
- Excellent customer service and communication skills.
- Knowledge of medical terminology.
- Minimum of 2 years of previous experience in a similar position.
- Ability to read, write, and speak the English language.
