Job Summary

We are seeking a highly motivated and detail-oriented Care Navigator to join our team in the Outpatient Behavioral Health department. As a Care Navigator, you will play a critical role in providing patient navigation services, ensuring seamless transitions of care, and facilitating patient compliance with ambulatory care, HIV care, and other services.

Key Responsibilities:
  • Manage transitions of care for patients from ED, inpatient, SNF, specialty to primary care
  • Address barriers to care coordination, improve patient experience, and connect patients to care as appropriate
  • Maintain tracking and documentation of referrals to promote team awareness and ensure patient safety
  • Assemble patient clinical background and referral needs, providing clinical information as applicable
  • Contact review organizations and insurance companies to ensure prior approval requirements are met
  • Act as an extender for the care team, assisting patients with problem-solving and facilitating care coordination and support plans
  • Identify and utilize cultural and community resources, serving as a liaison for internal and external care providers and families/caregivers
  • Track individual patient referral process, ensuring appointment scheduling, follow-up, and barrier resolution
  • Use motivational interviewing and teach-back tools to assist patient understanding of care plans
Requirements:
  • High School Diploma or GED required
  • 1-3 years' experience in a clinical care setting required
  • Intermediate knowledge of community resources needed for reentry at Denver Health's integrated care system
  • Ability to support community engagement and connect persons involved in the criminal justice system and Denver Health
  • Strong oral and written communication skills
  • Proven ability to work effectively with populations experiencing disadvantages, persons involved with the criminal justice system, and recent immigrants
