Care Navigator/Case Manager

We are seeking a highly motivated and detail-oriented Care Navigator/Case Manager to join our team at Webb Pediatric Clinic. As a key member of our healthcare team, you will play a vital role in ensuring seamless transitions of care for our patients, addressing barriers to care, and promoting patient engagement and retention.
Key Responsibilities:- Manage transitions of care for patients from emergency departments, inpatient settings, skilled nursing facilities, and specialty care to primary care.
- Identify and address barriers to care, including financial, social, and cultural challenges, to facilitate care coordination and improve patient outcomes.
- Maintain accurate tracking and documentation of referrals to ensure team awareness and patient safety.
- Assemble patient clinical background and referral needs, providing relevant clinical information as applicable.
- Communicate with review organizations and insurance companies to ensure prior approval requirements are met.
- Act as an advocate for patients and families, addressing concerns and facilitating care coordination.
- Collaborate with care teams to identify opportunities for improving patient care coordination and implement solutions.
- High School Diploma or GED required.
- 1-3 years of experience in a clinical care setting.
- Intermediate knowledge of community resources and the correctional and/or justice department(s).
- Strong oral and written communication skills.
- Ability to work effectively with diverse populations and provide empathetic care.
- Competitive salary range: $23.85 - $32.26 per hour.
