Key Responsibilities:
  • Evaluate patient care plans to ensure they align with established goals, issues, and nursing interventions.
  • Ensure all team members involved in patient care are informed of their respective care plans.
  • Organize and lead care plan meetings.
  • Oversee and facilitate the completion of required assessments.
  • Manage the Utilization Review (UR) process effectively.
  • Supervise medication administration records, treatment logs, and physician directives.
  • Engage in facility evaluations conducted by authorized regulatory bodies.
Qualifications:
  • Must hold a nursing degree from an accredited institution.
  • Must possess a valid, unrestricted nursing license in the relevant state.
  • MDS Certification is preferred.
  • Familiarity with the RAI process, CAAs, RUG categories, and MDS 3.0 is essential.
  • Understanding of nursing and medical practices, as well as relevant laws and regulations governing nursing facilities.
  • Ability to document nursing notes in a clear and detailed manner that reflects the care provided and the patient's response.
  • Experience with Electronic Medical Records and digital documentation systems.
  • Maintain confidentiality of all patient care information in compliance with HIPAA regulations.
