Responsibilities- Develop and Implement Care Plans: Review patient care plans to ensure they are accurate, comprehensive, and meet the individual needs of each resident.
- Collaborate with Healthcare Team: Work closely with nurses, doctors, and other healthcare professionals to ensure seamless care delivery.
- Monitor and Evaluate Care: Regularly assess the effectiveness of care plans and make adjustments as needed to ensure optimal outcomes.
- Manage Utilization Review: Oversee the utilization review process to ensure that care is delivered in a cost-effective and efficient manner.
- Maintain Accurate Records: Ensure that all patient records are accurate, up-to-date, and compliant with regulatory requirements.
- Nursing Degree: Possess a nursing degree from an accredited college or university.
- Current RN License: Hold a current, unencumbered, active license to practice as a registered nurse in this state.
- MDS Certification: Preferably certified in MDS.
- Knowledge of RAI Process: Knowledgeable and experienced in the RAI process, CAAs, RUG categories, and MDS 3.0.
- Excellent Communication Skills: Possess excellent communication and interpersonal skills to effectively collaborate with the healthcare team.
