Position:
Nurse Practitioner – PRN/Part Time

Assessment Process:
During a visit that can last up to one hour, our providers conduct a thorough review of a member's current health status, medical history, medication adherence, social environment, and other risk factors. This in-depth evaluation provides invaluable insights into a member's overall health, which can be challenging to capture during standard office visits.

Comprehensive Health Assessment:
The Matrix Comprehensive Health Assessment aims to enhance care quality and potentially address multiple care gaps in a single visit.

Travel Requirements:
Candidates must possess a valid state driver’s license and be able to travel as needed. Proof of adequate automobile insurance coverage is also required.

Collaboration:
Work collaboratively with physicians, case managers, social workers, family members, and other relevant medical personnel. Engage with Primary Care Physicians (PCPs) to provide patient education and follow-up care.

Technology Skills:
Strong computer skills and familiarity with health/medical record software are essential. Flexibility and adaptability to frequent changes are also required.
