Job Title: VA Case Manager - Grant Per Diem Program Lead

Join The Salvation Army Central Territory as a Lead Case Manager for our Veterans Housing Grant Per Diem program. As a key member of our team, you will oversee and coordinate the activities of veterans residing in our Transitional Housing Grant Per Diem programs.

Key Responsibilities:
  • Lead case management functions, including intakes, assessments, service plans, and progress notes, within required timelines.
  • Guide the case management team and provide support for productive day-to-day operations.
  • Manage an organized bookkeeping system for administration access while maintaining professional confidentiality.
  • Organize weekly classes and groups that incorporate life skills, employment skills, and community resources for clients.
  • Complete and record daily, weekly, monthly, quarterly, and annual activities, progress, outcome measurements, and reports.
  • Coordinate outreach, admission, and discharge with the Health Care for Homeless Veteran (HCHV) Grant Per Diem Liaison and staff.
  • Manage census and client accountability according to operating protocol.
  • Oversee case management engagement and follow up with case coordination to facilitate client goal achievement.
  • Utilize community partnerships and resources to enhance support for clients.
  • Participate in weekly staff meetings and supervision.
  • Provide updates and insights on client progress and resources needed to Program Directors.
  • Maintain updated client files and utilize recommended approaches and modules to support the GPD program.
  • Learn and maintain HMIS data information and other programming software.
  • Manage client daily living, including home living routine, cleaning, saving plan, and medication monitoring.
  • Review policies and procedures regularly and uphold The Salvation Army's mission and values.
Requirements:
  • Bachelor's degree in human service or social service field or related area, plus one year of work experience.
  • Experience working with recovering substance abusers and veterans is a plus.
  • Certified Alcohol and Drug Counselor (CADC) or Recognized Associate Substance Abuse Counselor (RASAC) or LPC or LCSW is a plus.
  • Excellent interpersonal and communication skills, understanding of the homeless and chemically dependent veteran population.
  • Ability to think independently and exercise sound judgment.
