Job Summary:The Outpatient Case Manager will conduct initial assessments, maintain case management responsibilities, and participate in staff training to learn skills such as interviewing, assessment, and establishing rapport with clients. You will also be responsible for maintaining client charts, ensuring quality care standards are followed, and acting as a client advocate to coordinate required services or resolve emergency problems.
Key Responsibilities:- Conduct thorough interviews with clients to gather information and formulate a diagnosis of psycho-social problems.
- Assess client status, including personal strengths, progress, problems, and needs.
- Participate in the development of diagnostically based treatment plans.
- Monitor client participation and progress in treatment programs to ensure planned services are being provided.
- Intercede on behalf of clients to assist them in achieving and maintaining community adjustment.
- Participate in case planning, supervisory conferences, and consultation to ensure effective delivery of services.
- Maintain productivity standards and ensure timely submission of billing logs.
We require a Master's degree in Social Work, Psychology, or Nursing, with a minimum of two years of post-graduate clinical experience in mental health counseling and psychotherapy. You must have a working knowledge of the DSM-III-R and diagnostic terminology.
