Responsibilities
  • Assess patients' discharge planning needs and develop individualized care plans in collaboration with the interdisciplinary team.
  • Identify and escalate gaps/barriers related to inpatient and observation length of stay, and develop strategies to mitigate these issues.
  • Participate in the utilization management process, ensuring that patients receive the most appropriate level of care.
  • Collaborate with social workers and other healthcare professionals to coordinate care and ensure a smooth transition to post-acute settings.
  • Develop and maintain relationships with post-acute providers, including skilled nursing facilities, rehabilitation centers, and home health agencies.
  • Stay up-to-date with regulatory requirements and industry best practices, ensuring compliance with Condition of Participation (CoP) discharge planning.
Requirements
  • Graduate of an accredited school of nursing.
  • Three years' acute care experience, preferably in a case management or utilization management role.
  • Registered Professional Nurse (RN) licensure in the state of Illinois, Missouri, Oklahoma, or Wisconsin.
