Job Summary

We are seeking a skilled Case Manager to join our team at Actalent. As a Case Manager, you will be responsible for synthesizing program goals and assessed needs to create individualized plans of care for clients. You will also arrange and coordinate service referrals, provide ongoing monitoring, and refer clients to public benefits programs as eligible.

Key Responsibilities
  • Case Management: Develop and implement individualized plans of care for clients, ensuring alignment with program goals and assessed needs.
  • Service Coordination: Arrange and coordinate service referrals for clients, including monitoring progress and evaluating the effectiveness of implemented activities.
  • Public Benefits: Refer clients to public benefits programs as eligible, ensuring access to necessary resources and support.
  • Caseload Management: Independently manage caseload, providing regular contact with enrolled clients and documenting all contacts and service coordination activities in client records.
Requirements
  • Education: Bachelor's degree in a health-related field, such as social work, counseling, sociology, or psychology.
  • Experience: Previous experience in case management, with a strong understanding of program goals and assessed needs.
