Job Title: Case Manager

The Salvation Army USA Central Territory is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a vital role in providing case management services to individuals and families involved in our program.

Key Responsibilities:
  • Conduct initial intake assessments to determine client needs and develop case plans to help families achieve self-sufficiency.
  • Provide case management services, including regular meetings and room inspections, to support clients in implementing their case plans.
  • Network with community agencies and organizations to enhance relationships and improve client services and referral systems.
  • Participate in staff meetings to present cases and develop individual case plans.
  • Maintain client information in a confidential manner and prepare residents for permanent or transitional housing.
  • Assist with transitional housing placement and maintain client files, including case notes and statistical data.
Requirements:
  • Bachelor's degree in a human services field or related work experience.
  • Outgoing personality with good communication skills and ability to work with diverse cultures.
  • Strong leadership style and good organizational skills.
  • Commitment to carrying out The Salvation Army's mission.
  • Knowledge of The Salvation Army and government, state, and community resources.
  • Working knowledge of computers and reliable transportation.
Work Environment:

The Case Manager will work in an office environment, using a computer, answering phones, and performing other administrative tasks. The position requires occasional lifting of up to 40 pounds and the ability to move 50 or more pounds using a dolly/cart/truck.
