About the Role

The Southwest Center for HIV AIDS INC is seeking a highly skilled and compassionate Medical Assistant Care Coordinator to join our team. As a Medical Assistant Care Coordinator, you will play a vital role in providing comprehensive care coordination services to our patients.

Key Responsibilities
  • Comprehensive Care Coordination: Collaborate with interdisciplinary teams to develop personalized care plans, manage referrals and prior authorizations, and ensure patients receive the appropriate resources and support throughout their healthcare journey.
  • Patient Support: Assist support staff and providers in establishing and coordinating care services, ensure seamless transitions between different levels of care, and facilitate effective communication among all stakeholders.
  • Strong Patient-Provider Relationships: Foster strong relationships with patients and providers, ensuring appropriate follow-up care and improving patient outcomes.
  • Administrative Tasks: Prepare for patient appointments, coordinate appointments for specific care and diagnostic procedures, report communicable diseases to agencies as required, and initiate and manage prior authorizations for medications and procedures.
  • Lab and Diagnostic Tests: Track laboratory and diagnostic tests, review patient gaps in care reports to ensure appropriate follow-up of required patient care, and contact and schedule patients identified.
  • Communication and Problem-Solving: Assess challenges, situations, or issues presented and appropriately prioritize while assisting with resolution using strong critical thinking and problem-solving skills.
  • Team Collaboration: Participate in orientation, mentoring, and training of new and established staff, and work with various healthcare and support staff to assure smooth and efficient patient flow and clinical operations.
Requirements
  • Experience: 2 or more years of experience as a Medical Assistant.
  • Education: High school diploma or completed GED, and a certificate of completion or diploma from an approved and accredited medical assisting training program.
  • Cultural Competency: Demonstrated cultural competency to professionally and respectfully serve the LGBTQIAS+ communities.
  • Skills: Excellent computer skills with proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook) and EHR software, highly organized, detail-oriented, and a self-starter with excellent problem-solving skills.
  • Interpersonal Skills: Strong interpersonal skills in dealing with a variety of people and proven ability to work independently while collaborating with others.
Preferred Qualifications
  • Experience with Patient-Centered Medical Home Models: Experience working in Patient-Centered Medical Home models of care.
  • Integrated Care Settings: Experience working in Integrated Care settings.
  • eClinicalWorks: Experience working with eClinicalWorks.
  • Bilingual: Bilingual in Spanish and English.
Environmental Factors, Physical Requirements, and Compliance
  • Work Environment: Work primarily in a climate-controlled environment with minimal safety/health hazard potential.
  • Travel: Travel outside of the Phoenix region approximately 10% of the time.
  • Certifications: Currently have, or be able to obtain within 90 days of employment, a valid Fingerprint Clearance Card, and currently have, or are able to obtain within 7 days of employment, a clear TB test, current flu vaccination, and Hepatitis-B primary vaccination series.
