About the Role:

Ovation Primary Care & Hospice is seeking a skilled and compassionate Certified Nursing Assistant (CNA) to join our team in providing exceptional care to our patients.

Responsibilities:

  • Provide personal care to patients under the direction of a registered nurse, ensuring their physical and emotional needs are met.
  • Support the operations of Ovation Primary Care & Hospice in the East Valley region, working collaboratively with other healthcare professionals to deliver high-quality patient care.
  • Document care provided according to organization policy, maintaining accurate and detailed records of patient interactions.
  • Report any deviations from the Plan of Care to the Case Manager, ensuring seamless communication and coordination of patient care.
  • Use Durable Medical Equipment safely, taking vital signs when on the Hospice Aide Plan of Care, and providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence.
  • Foster a cooperative effort among personnel by understanding the functions of other persons involved in patient care and actively participating in team and staff conferences.
  • Provide respite for patients' family/caregiver when on-site, as appropriate, and participate in Quality Assurance and Performance Improvement (QAPI) activities.

Requirements:

  • Meet training requirements by the state of Arizona.
  • One year experience as a CNA, hospice aide, or home health aide.
  • At least 18 years old.
  • Must be a licensed driver with an insured automobile in good working order.
  • The ability to communicate well, both verbally and in writing.
  • Ability to work as a team and also independently.
