Position Overview:
The Care Coordinator plays a crucial role in the Person Centered Services Care Coordination Organization, focusing on individuals with intellectual and developmental disabilities. This position emphasizes a holistic and individualized approach to care, ensuring that each person's needs and preferences are prioritized.

Key Responsibilities:

  • Conduct thorough assessments utilizing person-centered planning techniques and integrate relevant evaluations.
  • Create a detailed, person-centered Life Plan in collaboration with the individual and their support network, ensuring their active participation in the process.
  • Facilitate regular reviews of the Life Plan with the interdisciplinary team, particularly after significant life changes.
  • Oversee all aspects of an individual's care, ensuring effective management of a diverse caseload tailored to specific needs.
  • Maintain compliance with documentation and billing requirements, ensuring timely completion of necessary paperwork.
  • Engage with individuals in various settings to conduct assessments and provide essential services.
  • Coordinate with service providers and support teams to enhance care delivery.
  • Monitor individual satisfaction and ensure adherence to regulatory standards.
  • Promote a respectful and inclusive environment for both individuals and colleagues.
  • Perform additional duties as assigned by the Care Coordinator Supervisor.

Qualifications:

  • Bachelor's Degree with 2 years of relevant experience, or a Licensed Registered Nurse with 2 years of relevant experience, or a Master's Degree with 1 year of relevant experience.
