KEY RESPONSIBILITIES

As a Care Coordinator, you will collaborate to ensure that Health Home clients receive thorough care coordination. Within the Children's Health Home Team, you will oversee all aspects of care for child and family clients, which includes scheduling and tracking appointments, partnering with treatment providers, advocating for clients, and coordinating community services.

  • Manage services for assigned children's caseload based on needs assessment (High, Medium, or Low).
  • Conduct home visits and engage with clients in various community settings, including medical appointments and hospitals.
  • Utilize assessment tools such as CANS and HCS (Health Commerce System) to connect clients with necessary services.
  • Document consent on relevant forms as required.
  • Advocate for client rights and facilitate access to social services, mental health support, and other resources.
  • Perform comprehensive assessments and create a client-centered Plan of Care to meet individual needs.
  • Develop crisis plans with clients and review them annually with clients and caregivers.
  • Regularly review cases with clients and care team members to address barriers to achieving Plan of Care goals.
  • Complete monthly billing questionnaires for each client by the end of each month.
  • Coordinate care across the system, assisting clients in accessing health services and overcoming barriers.
  • Work directly with clients, families, and discharge planners to facilitate transition planning.
  • Assist clients and caregivers with appointment coordination, including scheduling and transportation.
  • Collaborate with the interdisciplinary care team, including primary care providers and mental health specialists.
  • Engage with educational institutions to ensure clients' needs are met.
  • Continuously enhance knowledge of community resources and services.
  • Follow up with inpatient facilities and emergency rooms upon client admission and ensure connections to aftercare.
  • Provide culturally sensitive care coordination services to a diverse clientele, advocating for language access as needed.
  • Document all client interactions and engagement efforts accurately, submitting progress notes promptly.
  • Utilize electronic systems effectively, including Netsmart and HEALTHeLink.
  • Maintain ongoing communication with program management regarding caseloads and seek support when necessary.
  • Participate in agency training and meetings as required.
  • Establish and maintain appropriate professional boundaries.

QUALIFICATIONS

  • Bachelor's Degree in a relevant field; social work or behavioral sciences preferred.
  • Two or more years of experience in community-based client services or a related area.
  • Experience in care coordination or management is highly desirable.
  • Familiarity with electronic health record systems is advantageous.
  • Knowledge of community resources is beneficial.
  • Ability to provide effective care management services with a supportive approach.
  • Willingness to conduct home visits and transport clients as needed.
  • Flexibility to work outside regular hours to meet client needs.
  • Proficiency in Microsoft Office and web-based health information systems.
  • Bilingual in English and Spanish is a plus.

This role requires travel between locations, necessitating a reliable vehicle, valid driver's license, and appropriate insurance. Proof of Covid-19 vaccination is also required upon hiring.


WHAT WE PROVIDE

  • Competitive hourly wage ranging from $21.00 to $28.00, based on experience and qualifications.
