Role Summary: The Care Coordinator plays a pivotal role in collaborating with the Care Manager Team to deliver comprehensive care coordination for Health Home consumers. This position is tasked with overseeing all facets of care for clients, which includes scheduling and managing appointments, liaising with treatment providers, advocating for clients, and orchestrating various community services.

Key Responsibilities:

  • Oversee and coordinate services for an assigned caseload.
  • Analyze client assessments and engage clients in identifying and addressing barriers to their health services and self-management.
  • Collaboratively develop an Individualized Care Plan (ICP) with clients, their families, and service providers under supervision.
  • Facilitate care coordination across the healthcare system, assisting clients in accessing necessary medical and social services.
  • Work closely with clients, families, and discharge planners to support effective transition planning.
  • Provide translation and interpretation services as needed.
  • Continuously enhance knowledge of community resources and services available.
  • Maintain professional boundaries and role definitions.
  • Conduct meetings with clients in various settings to introduce programs and perform necessary assessments.
  • Ensure timely completion of documentation and maintain accurate consumer records using electronic systems.

Qualifications:

  • Bachelor's Degree in a relevant field; social work or a related behavioral discipline preferred.
  • Minimum of 2 years of experience in community-based client services or equivalent.

Preferred Skills:

  • Familiarity with health record management systems.
  • Experience in care coordination or discharge planning is highly desirable.
  • Strong ability to provide effective care management services and demonstrate understanding of community resources.
  • Willingness to engage with the community, conduct home visits, and provide transportation for clients.
  • Proficiency in Microsoft Office applications and web-based health information systems.
  • Flexibility to work outside standard hours, including evenings and weekends as necessary.
  • Bilingual skills, particularly in Spanish, are highly valued.
