Job Title: Quality Improvement Facilitator

The Rutland Regional Medical Center is seeking a highly skilled Quality Improvement Facilitator to join our team. As a grant-funded position, this role will play a critical part in coaching primary care and specialty practices in the transformation to become certified NCQA Patient Centered Medical Homes.

Key Responsibilities:

  • Coach primary care and specialty practices in the transformation to become certified NCQA Patient Centered Medical Homes.
  • Work with local Blueprint Teams and Accountable Care Organizations (ACOs) to support community-based Quality Improvement efforts.
  • Assist healthcare teams in assessing, prioritizing process improvements, developing and implementing action plans, and evaluating improvements.

Requirements:

  • Bachelor's degree in a healthcare-related field.
  • Master's degree preferred.
  • 5 years of experience in a clinical, education, or administrative role with a focus on quality improvement.
  • 3 years of experience leading clinical quality improvement initiatives.
  • Prior experience working within Clinical Microsystems framework desirable.

Preferred Qualifications:

  • Quality Certification (CPHQ, CHQ, or HCQM) highly desirable.
  • Lean or Six Sigma Certification preferred.
  • Maintains a valid Vermont state driver's license.
  • Current Vermont professional licensure (as applicable).

Salary Range: $41.92
