The VNA & Hospice of the Southwest Region is seeking a dedicated full-time Practical Nursing Specialist to become a vital member of our interdisciplinary Hospice team. This role, operating under the guidance of a Registered Nurse or Clinical Manager, is essential for delivering personalized, goal-oriented nursing care in alignment with medical directives.

Key Responsibilities:

  • Employing the nursing process to deliver exceptional care to designated patients.
  • Executing individualized care plans, ensuring active participation from patients and their caregivers.
  • Maintaining thorough documentation and effectively communicating visit outcomes and necessary actions to the RN.
  • Implementing medical care plans, nursing interventions, and procedures to guarantee safe and high-quality patient care.
  • Assessing various elements of the care plan's effectiveness, including identified issues, care delivered, and patient responses, to fulfill patient and caregiver needs and uphold care quality.
  • Ensuring timely and precise documentation of nursing services to facilitate seamless information sharing among the healthcare team, promoting ongoing quality care.
  • Providing education to patients and caregivers to empower informed decision-making regarding medications, treatment options, discharge instructions, and other relevant topics.
  • Identifying interdisciplinary patient needs and documenting them appropriately, while promptly communicating recommendations to the case manager.
  • Conducting visits efficiently, ensuring that they are not combined with routine appointments unless necessary.
  • Reporting patient incidents (such as infections or injuries) to the Quality Improvement Specialist in a timely manner.
  • Coordinating diverse aspects of patient care with physicians, providers, team members, and vendors to support the established care plan, ensuring safe and effective care for patients and families.
  • Participating in monthly peer review audits, providing and receiving constructive feedback in a professional manner.
  • Proactively organizing and facilitating case conferences with the Manager and team members to enhance communication and ensure effective documentation of meetings.

Qualifications:

  • Previous clinical acute care nursing experience is required, with a preference for prior Hospice experience.

Licensure:

  • Current Licensed Practical Nurse (LPN) licensure is mandatory.
  • A valid state driver's license is required, along with the ability to pass background checks including motor vehicle record, child abuse, criminal record, and adult abuse checks.
