The ideal candidate will have a strong background in healthcare quality improvement, clinical Microsystems, and facilitation. Key responsibilities include coaching healthcare teams in assessing and prioritizing process improvements, developing and implementing action plans, and evaluating outcomes. The successful candidate will work collaboratively with local Blueprint Teams and Accountable Care Organizations to support community-based quality improvement efforts. A Bachelor's degree in a healthcare-related field is required, with a Master's degree preferred. The ideal candidate will have at least 5 years of experience in a facilitator or related healthcare role, with 3 years of experience leading clinical quality improvement initiatives. Lean or Six Sigma Certification is highly desirable. The successful candidate will possess strong verbal and written communication skills, with the ability to work effectively with diverse professionals within multi-disciplinary primary care teams. Salary Range: $41.92
