Healthcare Quality Improvement Specialist

The Rutland Regional Medical Center is seeking a skilled Quality Improvement Facilitator to join our team. As a grant-funded position, this role will play a critical part in coaching primary care and specialty practices in the transformation to become certified NCQA Patient Centered Medical Homes.
Key Responsibilities- Coach healthcare teams in assessing, prioritizing, and implementing process improvements
- Develop and evaluate action plans to improve patient outcomes and decrease costs
- Collaborate with local Blueprint Teams and Accountable Care Organizations to support community-based Quality Improvement efforts
- Bachelor's degree in a healthcare-related field; Master's degree preferred
- 5 years of experience in a clinical, education, or administrative role with a focus on quality improvement
- 3 years of experience leading clinical quality improvement initiatives
- Lean or Six Sigma Certification preferred
- Quality Certification (CPHQ, CHQ, or HCQM)
- Prior experience working within Clinical Microsystems framework
