Job Summary

The Rutland Regional Medical Center is seeking a skilled Quality Improvement Facilitator to join our team. As a grant-funded position, this role will play a critical part in coaching primary care and specialty practices in the transformation to become certified NCQA Patient Centered Medical Homes.

Key Responsibilities
  • Coach healthcare teams in assessing, prioritizing, and implementing process improvements
  • Develop and evaluate action plans to improve patient outcomes and decrease costs
  • Collaborate with local Blueprint Teams and Accountable Care Organizations to support community-based Quality Improvement efforts
Requirements
  • Bachelor's degree in a healthcare-related field; Master's degree preferred
  • 5 years of experience in a clinical, education, or administrative role with a focus on quality improvement
  • 3 years of experience leading clinical quality improvement initiatives
  • Lean or Six Sigma Certification preferred
Preferred Qualifications
  • Quality Certification (CPHQ, CHQ, or HCQM)
  • Prior experience working within Clinical Microsystems framework
