Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our team. As a RN Case Manager, you will be responsible for providing direct care to patients in their homes, while also overseeing and directing home health services.

Key Responsibilities
  • Provide skilled nursing care to patients in their homes, including assessments, interventions, and evaluations.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Provide education and support to patients and their families to promote health and wellness.
  • Manage and coordinate home health services, including scheduling and communication with healthcare providers.
  • Ensure accurate and timely documentation of patient care and services provided.
  • Participate in on-call rotation to provide 24/7 coverage for patients.
Requirements
  • Current licensure as a Registered Nurse (RN) in the State of Minnesota.
  • Proof of negative TB testing.
  • Access to a dependable automobile for travel to multiple business stops.
  • Valid driver's license and proof of current car insurance.
  • U.S. Citizenship or valid Alien Work Permit.
  • Background studies and screenings, including those of the Minnesota Department of Health and the Federal Office of the Inspector General's List of Excluded Individuals and Entities.
Preferred Qualifications
  • At least two years of experience in a healthcare facility or home care field.
  • Critical thinking skills and ability to follow standards/procedures.
  • Ability to work independently with minimal supervision and adapt to change.
  • Excellent written and oral communication skills.
  • Basic computer skills and willingness to learn new software programs.
