Responsibilities- Provide comprehensive nursing assessments, develop and implement individualized care plans, and evaluate patient outcomes.
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and communication.
- Manage a caseload of 25-30 patients, prioritizing their needs and providing timely interventions to ensure optimal health outcomes.
- Delegate tasks and supervise home health aides and homemakers to ensure efficient and effective care delivery.
- Participate in on-call rotations to provide 24/7 support to patients and families.
- Current RN licensure in the State of Minnesota.
- At least one year of medical-surgical or related nursing experience.
- Home health care experience preferred.
This role requires daily travel by automobile in all weather conditions and occasional exposure to unclean or unsanitary conditions during home visits.
Physical DemandsThis position involves sitting, driving, standing, walking, repeated bending, squatting, stooping, or similar strenuous duties, hand repetitive movements, and viewing a digital monitor. Lifting up to 50 pounds is occasionally required.
