Key Responsibilities
  • Provide one-to-one patient care in a home environment, executing physicians' prescribed plans of care and documenting care provided in a timely manner.
  • Administer prescribed medication, treatments, and therapies, and coordinate care to ensure seamless patient outcomes.
  • Conduct patient assessments, develop and implement nursing care plans, and recognize changes in patient needs and responses requiring intervention.
  • Collaborate with healthcare professionals to ensure comprehensive patient care, and educate patients and families regarding disease processes, self-care techniques, and prevention strategies.
  • Maintain knowledge of competencies related to the nursing profession, participating in educational programs and continued education units to enhance clinical skills and expertise.
Requirements
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current, unrestricted state license as a Registered Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
