Job SummaryWe are seeking a highly motivated and compassionate Registered Nurse to join our Hospital at Home team. As a Registered Nurse Case Manager, you will provide nursing care, care coordination, lab draws, nebulizer treatments, and other necessary cares for patients during their acute stay.
Responsibilities- Provide high-quality nursing care to patients in their homes, promoting independence and self-care.
- Coordinate care with other healthcare professionals to ensure seamless transitions and optimal patient outcomes.
- Conduct lab draws, administer medications, and perform other necessary procedures as required.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with patients, families, and caregivers to ensure comprehensive understanding of care plans and goals.
- Graduate from a CCNE or NLN accredited nursing program is strongly preferred.
- Bachelor's degree in nursing preferred.
- Minimum 2 years related experience and/or training; or equivalent combination of education and experience preferred.
- License as a Registered Nurse (RN) in South Dakota and/or meeting the requirements of Multi-state licensure is required.
- Valid driver's license and insurability by Avera Health automobile liability insurance carrier.
- Competitive hourly rate: $29.50 - $44.25 per hour.
