Job SummaryWe are seeking a skilled and compassionate RN Case Manager to join our team. As a RN Case Manager, you will be responsible for providing direct care to patients in their homes, while also overseeing and directing home health services.
Key Responsibilities- Provide skilled nursing care to patients in their homes, including assessments, interventions, and evaluations.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
- Provide education and support to patients and their families to promote health and wellness.
- Manage and coordinate home health services, including scheduling and communication with healthcare providers.
- Ensure accurate and timely documentation of patient care and services provided.
- Participate in on-call rotation to provide 24/7 coverage for patients.
- Current licensure as a Registered Nurse (RN) in the State of Minnesota.
- Proof of negative TB testing.
- Access to a dependable automobile for travel to multiple business stops.
- Valid driver's license and proof of current car insurance.
- U.S. Citizenship or valid Alien Work Permit.
- Background studies and screenings, including those of the Minnesota Department of Health and the Federal Office of the Inspector General's List of Excluded Individuals and Entities.
- At least two years of experience in a healthcare facility or home care field.
- Critical thinking skills and ability to follow standards/procedures.
- Ability to work independently with minimal supervision.
- Excellent written and oral communication skills.
- Basic computer skills and willingness to learn new software programs.
