Key Responsibilities

  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Conduct home visits to assess patients' living environments and provide recommendations for modifications to promote safety and accessibility.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Master's or Doctorate degree in Occupational Therapy from an accredited program.
  • Current licensure as an Occupational Therapist in the state of Florida.
  • Minimum of six months of relevant experience in home health or a related field.
  • Active Driver's License and reliable transportation.
  • Basic Life Support (BLS) certification.
