Position Overview

GREATER PHILADELPHIA URBAN AFFAIRS is looking for a dedicated Case Manager to enhance our community anti-violence initiative. This program aims to link individuals at elevated risk of gun violence with essential resources through trusted community representatives, specifically barbers. Our organization will provide training for barbers in various neighborhoods to help them identify clients who may be vulnerable to gun violence and facilitate meaningful conversations that connect them to our case management services or other necessary support.

The Case Manager will be responsible for managing the hotline and responding to inquiries from barbers, community members, and youth. This role involves educating individuals about available programs and resources, as well as assisting them in navigating the complexities of accessing these services. While some may only require information about a program, others will benefit from personalized support to overcome obstacles. For instance, the Case Manager may assist individuals in completing online applications, contacting agencies, or gathering required documentation.

When a young person is referred to our organization by a barber, family member, or community member, the Case Manager will collaborate with the youth and their support network to assess their needs and create a tailored plan to eliminate barriers to service access. The Case Manager may enroll youth in existing programs or provide referrals to partner organizations. Additionally, follow-up will be conducted to ensure that youth are receiving the necessary services and to address any challenges that may arise.
