The Community Support Coordinator at GREATER PHILADELPHIA URBAN AFFAIRS plays a vital role in providing integrated case management for individuals affected by violence and their families. This position emphasizes the importance of connecting clients with essential resources, assisting them in achieving their objectives, and ensuring a high level of satisfaction with the services provided.

Qualifications, Skills, and Competencies:
  • Experience in providing trauma-informed care for individuals impacted by violence
  • Strong interpersonal and communication skills
  • Understanding of diverse communities and available local resources
  • Familiarity with interdisciplinary teams in healthcare or Violence Intervention Programs
  • Proficient in documentation, decision-making, and managing client relationships
  • Ability to resolve conflicts effectively
Key Responsibilities:
  • Evaluate client needs and monitor their progress
  • Create and oversee individualized treatment plans
  • Facilitate access to services and resources
  • Advocate on behalf of clients
  • Maintain accurate client records
  • Report on client progress during team meetings
  • Prepare and assess discharge plans
  • Balance professional responsibilities with learning opportunities
  • Manage budgets and assist with grant proposals
  • Promote the organization’s reputation within the community

Education, Experience, and Licensing Requirements:

  • Associate degree required; Bachelor’s degree preferred
  • Bilingual candidates are encouraged to apply
  • Reliable transportation is necessary
  • Background checks are mandatory
  • Prior experience in case management is preferred
  • Customer service experience and a valid driver’s license are required
  • Candidates must be at least 21 years of age
