Key Responsibilities:
  • Engage proactively with patients managing chronic conditions to verify benefits and assist in identifying suitable secondary coverage options.
  • Conduct insurance verifications and medication coverage assessments, collaborating with healthcare providers to address formulary limitations.
  • Facilitate the completion of prior authorizations or appeals necessary for securing medication access for patients.
  • Maintain regular communication with patients regarding their current medication usage and ongoing requirements, liaising with in-house or contracted pharmacies.
  • Perform routine reassessments of patients' medication usage, identifying any concerns or side effects they may encounter.
  • Consult with pharmacists or healthcare providers regarding all clinical needs of patients, ensuring all clinical interventions or counseling are routed to a licensed clinician.
  • Assist patients in scheduling prescriptions, refills, or deliveries.
  • Document all patient interactions within specialty pharmacy software.
  • Generate regular reports to identify new patients, upcoming medication needs, and adherence-related data.
  • Support patients attending in-person visits by collecting vital signs and updating relevant health information.
  • Ensure patients receive necessary labs during in-person visits, schedule follow-up appointments, and provide printed orders for additional tests.
  • Administer health questionnaires to patients as needed, utilizing tablets and other technologies.
  • Guide patients in installing and accessing the AndHealth app on their mobile devices.
  • Manage patient referrals for specialty care.
  • Perform additional responsibilities as required.
Education & Licensure Requirements:
  • Bachelor's degree is preferred.
  • Certification as a Pharmacy Technician in the state of OH is required.
  • National certification - PTCB or ExCPT is preferred.
  • Willingness to obtain and maintain licenses or certifications across multiple states is essential.
Preferred Skills and Qualifications:
  • Minimum of 3 years of experience in pharmacy or nurse navigation is preferred.
  • Exceptional written and verbal communication skills.
  • Strong attention to detail with the ability to manage multiple tasks simultaneously.
  • Comfortable with technology and adaptable to new systems.
  • Willingness to travel as needed, with reliable transportation.
  • Organized and capable of working independently.
Additional Requirements:
  • Proof of negative TB skin test within the last 12 months.
  • Documentation of MMR immunity or a blood titer.
  • Proof of Varicella vaccination (2 documented doses or a blood titer).
  • Completion of Hepatitis B series.
  • Influenza vaccination is required.
  • COVID vaccination is required.
