We are seeking a Care Navigator to fulfill a vital role at AndHealth. In this position, you will support patients throughout their healthcare journey, aiming to achieve exceptional health outcomes and ensuring they receive the necessary care for a healthier lifestyle.
You will collaborate with our Physicians, Nurses, Health Coaches, and Patient Services team, providing insights on how to enhance the patient experience.
Key Responsibilities:
- Prepare examination rooms for patient care by ensuring they are adequately stocked and sanitized.
- Manage patient communications, receive and route test results for provider review, and gather information regarding patient medications.
- Room patients, monitor vital signs, and assess patient needs to relay to the provider.
- Assist the provider with patient procedures as directed.
- Communicate effectively, both verbally and in writing, with patients, their families, colleagues, and external entities.
- Complete necessary forms for patients as required.
- Ensure timely and accurate documentation in the Electronic Health Record (EHR).
- Conduct laboratory tests as instructed by the provider, prepare specimens, operate lab equipment, and maintain laboratory records in compliance with standards.
- Reconcile billing from external laboratories by comparing daily lab logs with patient EHR.
- Schedule external diagnostic tests and communicate effectively with patients.
- Facilitate referral processes for patients as necessary.
- Conduct manual inventory of medications.
- Perform front office tasks as required.
- Adhere to all policies and procedures of the community health center.
- Other duties as assigned based on organizational needs.
- Bachelor's degree is preferred.
- LPN is preferred.
- RN is preferred.
- American Heart Association Basic Life Support Provider (BLS) certification required or willingness to obtain.
- Experience in an outpatient or ambulatory care setting is preferred.
- Proficiency in Microsoft Office applications.
- Ability to navigate Electronic Medical and Health Record (EMR, EHR) systems.
- A strong sense of ownership and initiative to enhance each patient's experience.
- Excellent written and verbal communication skills.
- Attention to detail and the ability to manage multiple tasks simultaneously.
- A servant-leadership approach in interactions with all individuals.
- Comfort with technology and eagerness to learn new systems.
- Ability to thrive in collaborative environments with diverse teams.
- Proof of negative TB skin test within the last 12 months.
- Documentation of immunity to Measles, Mumps, and Rubella or a blood titer.
- Documentation of two doses of Varicella or a blood titer.
- Completion of Hepatitis B series.
- Influenza vaccination required.
- COVID vaccination required.
