Key Responsibilities:
- Serve as the lead clinical expert at the specialty pharmacy.
- Offer clinical direction and supervise clinical decision-making for the specialty pharmacy initiative.
- Ensure clinical oversight for the services and operations provided at the specialty pharmacy.
- Take charge of the pharmacy's Quality Management Committee and manage the Quality Management Program.
- Initiate, supervise, and report on Quality Improvement Projects across pharmacy locations.
- Deliver outstanding patient care and support to health center providers.
- Actively engage in clinical pharmacy services as part of the Patient Management Program.
- When applicable, provide specific clinical services through established protocols or collaborative practice agreements.
- Deliver clinical services, therapeutic advice, and counseling to patients managing chronic conditions such as asthma, dermatitis, epilepsy, and more.
- Regularly review and stay updated with clinical guidelines and literature essential for effective specialty care management.
- Serve as a resource for providers and patients seeking health and medication information.
- Conduct thorough reviews of patient therapies for duplications, interactions, and potential allergies.
- Recommend medication alternatives, consolidation, and dosage adjustments in line with current clinical guidelines.
- Collaborate with Engagement Specialists to ensure continuous medication access for all patients.
- Supervise Specialty Pharmacists, ensuring they receive the necessary training and resources for clinical excellence.
- Review payer formularies and manage prior authorizations and appeals as needed.
- Document all required information in specialty pharmacy software to maintain accreditation standards.
- Participate in URAC and ACHC specialty pharmacy accreditation preparations and visits as required.
- Mentor pharmacy interns and students.
- Undertake additional responsibilities as assigned.
Qualifications:
- PharmD, BS in Pharmacy, or equivalent FPGEC (NABP) required.
- Unrestricted pharmacist license in good standing in the state of Ohio.
- Willingness to obtain and maintain licenses in multiple states.
Preferred Skills:
- Minimum of 3 years of direct patient care experience in pharmacy.
- BLS/CPR certification required (or must be obtained within one month of hire).
- Immunization delivery certification preferred.
- 2+ years of specialty pharmacy experience preferred.
- PGY1 residency or equivalent experience preferred.
- CSP credential preferred.
- Exceptional written and verbal communication skills.
- Strong attention to detail and ability to manage multiple tasks simultaneously.
- Proficiency with technology and adaptability to new systems.
- Willingness to travel as needed.
Additional Health Requirements:
- Proof of negative TB skin test within the last 12 months.
- Documentation of MMR immunity or blood titer.
- Varicella - two documented doses or blood titer.
- Hepatitis B series.
- Influenza vaccination required.
- COVID vaccination required.
- Additional health requirements may apply.
