We are seeking dedicated Pharmaceutical Care Advocates to fulfill a vital role at AndHealth. This position requires onsite engagement at our partner health center.
Key Responsibilities:
- Engage proactively with patients experiencing chronic health issues to verify benefits and assist in identifying suitable secondary coverage options.
- Conduct insurance verifications and medication coverage assessments, collaborating with healthcare providers to navigate formulary limitations.
- Facilitate the completion of prior authorizations or appeals necessary for ensuring patient access to medications.
- Maintain regular communication with patients regarding their medication usage and ongoing requirements, liaising with in-house or contracted pharmacies.
- Perform routine reassessments of patients' medication use, identifying any concerns or side effects they may encounter.
- Consult with pharmacists or healthcare providers regarding the clinical needs of patients, directing all clinical interventions or counseling to a licensed clinician.
- Assist patients in scheduling their prescriptions, refills, or deliveries.
- Document all patient interactions within specialty pharmacy software.
- Generate regular reports to identify new patients, upcoming medication requirements, and adherence-related metrics.
- Support patients attending in-person visits by collecting vital signs and updating relevant health information.
- Ensure patients complete necessary lab work during visits, schedule follow-up appointments, and provide printed orders for additional tests.
- Administer health questionnaires to patients as needed, utilizing tablets and other technologies.
- Guide patients in installing and accessing the AndHealth app on their mobile devices.
- Manage patient referrals for specialty care.
- Perform additional responsibilities as required.
- A Bachelor's degree is preferred.
- Certification as a Pharmacy Technician in the state of Ohio is required.
- National certification (PTCB or ExCPT) is preferred.
- Willingness to obtain and maintain licenses or certifications across multiple states is necessary.
- A minimum of 3 years of experience in pharmacy or nurse navigation is preferred.
- Exceptional written and verbal communication skills.
- Strong attention to detail with the ability to manage multiple tasks simultaneously.
- Comfortable with technology and adaptable to new systems.
- Willingness to travel as required, with reliable transportation.
- Highly organized and capable of working independently.
- Proof of a negative TB skin test within the last 12 months.
- Documentation of MMR immunity or a blood titer.
- Evidence of two doses of Varicella or a blood titer.
- Completion of the Hepatitis B series.
- Influenza vaccination is required.
- COVID vaccination is required.
