Key Responsibilities:
  • Facilitates patient care within the Advocate Health system, ensuring seamless communication between departments as directed by healthcare providers.
    • 1) Coordinates services with team members to minimize patient wait times.
    • 2) Provides translation services between Spanish and English as necessary.
    • 3) Reinforces discharge instructions as outlined by the healthcare provider.
    • 4) Acts as a patient chaperone upon request from the provider.
    • 5) Undertakes additional duties as assigned.
    • 6) Assists patients in accessing further services as prescribed by the provider, including scheduling appointments.
    • 7) Alerts providers regarding patient needs and any changes in condition.
    • 8) Organizes activities aimed at enhancing patient satisfaction.
    • 9) Manages appointment scheduling with various healthcare facilities within the Advocate Health network.
  • Administrative Duties
    • 1) Reviews and updates appointment schedules, manages workflow, and informs providers of any delays or status changes.
    • 2) Maintains daily and monthly logs of patient visits.
    • 3) Completes necessary forms for patients and orders durable medical equipment as required.
    • 4) Ensures patient education materials are available in examination rooms and coordinates restocking as needed.
    • 5) Handles incoming calls and messages, directing them to the appropriate department or personnel.
    • 6) Prepares monthly summaries of visits for supervisory staff.
    • 7) Maintains logs for essential medical processes, updating them regularly to meet service needs.
    • 8) Reviews and organizes medical records for patient visits, ensuring all necessary information is available.
    • 9) Scans and uploads lab results and other documents into electronic patient records.
    • 10) Ensures a clean and organized work environment.

Education and Experience Requirements:
  • High school diploma or GED is required. Familiarity with medical terminology is preferred. A minimum of 1-2 years of experience in an ambulatory care setting or a related field is desirable.
Knowledge, Skills, and Abilities Required:
  • Strong communication skills, proficiency with computers, and effective time management and organizational abilities.

Physical Requirements and Working Conditions:
  • Ability to work flexible shifts to accommodate the needs of the health center. Must be able to work under pressure and in challenging situations. Capability to lift up to 50 pounds frequently and/or 10-20 pounds consistently. Must respond promptly to patient needs and maintain a fast-paced work environment. The role may involve exposure to hazardous or unpleasant conditions while providing care. Ability to assist in patient evacuation during emergencies is essential.
  • If the position involves direct patient care or contact, the following lifting requirement applies: Ability to lift up to 35 pounds without assistance. For lifts exceeding 35 pounds, patient handling equipment should be utilized, with assistance from at least one other staff member when available. Unique lifting scenarios will be evaluated individually.
