Job Opportunity

Fortcom Health Services is seeking a Part-Time Care Manager to join our team in the North Shore area of Massachusetts. As a Care Manager, you will be responsible for conducting home visits to members of our Adult Foster Care Program, ensuring their needs are met and their care plans are implemented effectively.

Responsibilities
  • Conduct thorough psychosocial evaluations to assess members' suitability for Adult Foster Care
  • Select, train, and supervise AFC caregivers in collaboration with our registered nurses
  • Develop, implement, and review AFC plans of care in conjunction with our interdisciplinary team
  • Perform regular on-site visits with members at their qualified settings
  • Review AFC caregiver logs at least every 30 days
  • Report changes in members' conditions to their AFC registered nurses
  • Participate in discharge and transition planning and implementation
Requirements
  • Bachelor's degree and two years of clinical experience in elder care or disability services
  • Massachusetts social worker license
