Position Overview:
As a key member of our team at Pca Care Connections, the Service Coordinator I plays a vital role in facilitating and coordinating long-term services and supports. This position reports directly to a Supervisor and employs a person-centered methodology to enhance psychosocial and health outcomes for individuals with disabilities and older adults.

Key Responsibilities:
  • Conduct comprehensive assessments using the InterRAI and Person-Centered Planning Tool (PCSP) to evaluate participants' needs, strengths, preferences, and goals.
  • Collaborate with participants, their families, and caregivers to create tailored service plans, consulting with other professionals as necessary.
  • Identify and mobilize both informal and formal resources to fulfill participants' needs, including long-term services and supports, as well as community services.
  • Facilitate participant choice in selecting providers and Service Coordinators.
  • Ensure compliance with all Managed Care Organization (MCO) required timelines and documentation standards.
  • Participate in Interdisciplinary Team meetings to enhance service delivery.
  • Conduct home visits and phone contacts to monitor the effectiveness of service plans and track goal achievement.
  • Support transitions of care and coordinate with behavioral health services.
  • Monitor participant satisfaction to guarantee high-quality service delivery.
  • Assist participants in maintaining their eligibility for Community Health Choices (CHC).
  • Complete critical incident reports and root cause analyses as necessary.
  • Inform participants of their rights and assist with complaint and grievance processes.
  • Adhere to HIPAA regulations and utilize a tablet for fieldwork to complete necessary documentation.
  • Engage in quality improvement initiatives and participate in ongoing training and professional development.

Qualifications:
  • Education: A Bachelor's degree in Social Work, Psychology, or a related field is required.
  • Experience: A minimum of three years of experience in a social service or healthcare environment is preferred.

Additional Requirements:
  • A valid driver's license and a good driving record are essential, along with continuous access to a fully insured vehicle.
  • Successful completion of drug testing and a pre-employment physical is required.
  • Proficiency in a second language is preferred.

Salary: The salary for this position is $43,789.77, commensurate with experience.
