The Agency on Aging of South Central Connecticut (AOASCC) is a dedicated non-profit organization focused on supporting older adults, individuals with disabilities, and caregivers in the greater New Haven area and the lower Valley.

As a Service Navigator, you will play a crucial role in the Aging Answers team. Your responsibilities will include providing tailored assistance to individuals and groups, primarily through telephone communications.

You will support clients and caregivers in accessing vital services related to health insurance, Medicare, Medicaid, Long-Term Care, and various community resources.

The objective is to relieve pressure on existing staff, improve workflow efficiency, and prevent costly interventions such as institutional placements.

Complex Navigation Support:
Assist clients in maneuvering through intricate systems related to health and social services. This encompasses understanding and clarifying eligibility criteria, application procedures, and available options.

Application Assistance:
Aid clients in completing necessary forms and applications for diverse programs.

Case Management:
Deliver case management services, ensuring continuity of care and timely follow-up.
Provide informed guidance on available services, empowering clients to make well-informed choices.

Short-Term Support:
Address urgent needs swiftly, ensuring timely interventions.
A master's degree in social work or a related field in Human Services is highly recommended.

AIRS Certification:
The Service Navigator should either possess AIRS certification or be eligible to take the certification exam.

SHIP/CHOICES Certified:

Familiarity with SHIP (State Health Insurance Assistance Program) and CHOICES (Connecticut's program for Health insurance assistance, Outreach, Information, Counseling, and Eligibility Screening) is beneficial.
