Social Services Program Facilitator

Job Overview
In this pivotal role, you will collaborate closely with the Program Supervisor to assist in the assessment and enrollment of clients into various social service initiatives. Your responsibilities will encompass delivering exemplary case management, advocacy, and referral services to our esteemed clients. Additionally, you will ensure meticulous documentation of services while building partnerships with governmental bodies and community organizations.
Key Responsibilities
- Provide outstanding customer service and guide clients in accessing advocacy and community resources.
- Assess client eligibility for programs and facilitate their enrollment in appropriate services tailored to their requirements.
- Collect essential documentation for evaluations and enrollment processes.
- Execute comprehensive case management by identifying client needs, developing service plans, and empowering clients towards independence.
- Design and disseminate promotional materials, coordinate outreach initiatives, and enhance resource networks for the Social Service Department.
- Maintain precise records of services rendered, adhering to all organizational policies and contractual commitments.
- Deliver accurate service data and reports to the Lead/Senior Case Manager and funding entities.
- Pursue personal development objectives and engage in ongoing training for professional advancement.
- Assist with front desk responsibilities as necessary.
- Participate in workshops, training sessions, and agency events as required.
- Perform any additional tasks as assigned.
Qualifications
- A Bachelor's degree in Social or Behavioral Sciences, or a related discipline, from an accredited institution, or a minimum of two years' experience in social services.
- Possess knowledge of community resources and a strong understanding of social service methodologies.
- Bilingual proficiency in English and either Chinese, Vietnamese, or Spanish is required.
- Exhibit strong organizational abilities, attention to detail, and the capacity to work autonomously.
- Demonstrate excellent communication and interpersonal skills, with a talent for teamwork.
- Willingness to work on weekends as needed.
- Ability to engage with a diverse clientele and establish rapport with individuals and families.
- Possess a valid California Driver's license, auto insurance, and reliable transportation.
