Role Summary
The Case Manager plays a crucial role in collaborating with a diverse team of social service professionals to ensure the effective delivery of care that aligns with the mission of St. Joseph Center. This position focuses on supporting individuals identified by the Office of Diversion and Reentry, facilitating their transition from incarceration to stability through comprehensive case management.

Key Responsibilities

  • Establish and nurture relationships with community stakeholders, law enforcement, and other service providers.
  • Assist clients in accessing interim housing solutions and guide them through the necessary enrollment processes for permanent housing programs.
  • Collaborate with various service providers to ensure coordinated care for clients.

Essential Duties

  • Respond promptly to referrals and complete all necessary tasks prior to client release.
  • Provide tailored case management and support services to address individual client needs.
  • Build trust with clients experiencing homelessness and conduct assessments using the Vulnerability Index (VI) SPDAT.
  • Facilitate connections to physical and mental health services, as well as assist clients in obtaining identification and mainstream benefits.
  • Maintain organized client files and accurately input data into the Homeless Management Information System (HMIS).
  • Provide transportation assistance as needed, including bus tokens and taxi vouchers.
  • Engage in case conferencing and community meetings to enhance service delivery.
  • Coordinate outreach efforts with other teams to maximize impact.
  • Ensure regular engagement with clients in various settings, including jails and homeless encampments.

Qualifications

  • Knowledge, Skills & Abilities: Must be self-motivated with strong communication skills to engage with a diverse population. Excellent organizational skills are essential for managing a fast-paced environment.
  • Experience: Minimum of two years working with vulnerable populations.
  • Education: Bachelor's degree in Psychology, Social Work, Human Services, or a related field from an accredited institution.
  • Technical Skills: Proficiency in MS Office Suite and experience with electronic health records.
