Case Manager

Position Overview: The Case Manager is essential in assisting individuals experiencing homelessness. This role involves supporting clients through various phases of their journey, including outreach, temporary housing, and securing permanent accommodations. The Case Manager will offer tailored assistance to ensure clients gain access to vital services such as healthcare, social support, and community resources.



Key Responsibilities:

  • Client Support: Guide clients through all stages of homelessness, providing personalized assistance to help them identify their strengths, challenges, health requirements, and housing aspirations.
  • Service Coordination: Aid clients in navigating healthcare and social service systems. Formulate and execute a detailed care plan focusing on housing and health objectives.
  • Assessment & Planning: Perform intake evaluations and develop Individualized Service Plans (ISP) that address housing goals, income enhancement, and other personal targets such as medical, mental health, substance use, financial resources, vocational, and social support needs.
  • Resource Connection: Facilitate referrals and link clients to resources that align with their ISP objectives. Conduct program-specific assessments and periodic reviews to monitor progress and modify plans as necessary.
  • Documentation & Compliance: Ensure accurate and timely maintenance of case files and client records in line with organizational policies. Confirm that all necessary documentation is up-to-date and complete.
  • Collaboration & Teamwork: Collaborate effectively with colleagues, clients, and stakeholders to achieve shared goals. Cultivate positive working relationships and acknowledge the contributions of others.
  • Communication: Convey information clearly and effectively to individuals and groups, both verbally and in writing, to promote understanding and engagement.
  • Critical Thinking & Problem Solving: Evaluate information critically to make informed decisions that lead to desired outcomes. Address challenges with logical reasoning and sound judgment.
  • Ethics & Boundaries: Maintain ethical and professional standards, demonstrating integrity while upholding professional boundaries.
  • Driving: Travel as necessary to meet clients and access resources is a fundamental aspect of this role.

Qualifications:

  • Demonstrated experience in case management or a related field.
  • Strong knowledge of homelessness and the necessary services to address it.
  • Excellent communication and interpersonal abilities.
  • Capacity to work collaboratively within a team setting.
  • Proficient in critical thinking and problem-solving skills.
  • Commitment to upholding ethical standards and professional boundaries.
  • Valid driver's license and dependable transportation.

Preferred Skills:

  • Familiarity with local resources and support systems.
  • Experience with program-specific assessments and documentation.
