Care Coordination Expertise

Cayuga Health System is seeking a skilled LMSW Case Manager to join our Inpatient Units team. As a key member of our care coordination team, you will play a vital role in delivering comprehensive care to patients and their families.

Key Responsibilities:
  • Evaluate hospital admissions to ensure medical necessity and appropriate level of care
  • Communicate effectively with providers and care teams to ensure accurate documentation and patient status
  • Participate in multidisciplinary rounds to drive patient-centered care
  • Issue federal and state notices as required
  • Serve as discharge planner for assigned patients
  • Assess patient risk for readmission and identify community support systems
  • Ensure patient demographics, insurance, and advanced directives are up-to-date in the EMR
  • Facilitate referrals to support teams and community-based organizations
  • Work to eliminate barriers to discharge
Requirements:
  • Master's degree in Social Work (LMSW)
  • Case Management Certification (or equivalent) within 18 months of hire
  • Minimum 1-3 years' experience in an acute care hospital setting or similar healthcare experience
  • Knowledge of community-based organizations and strong communication, organizational, critical thinking, and problem-solving skills
  • Computer literacy and knowledge of EMR systems
Preferred Qualifications:
  • Case Management Certification
Physical Requirements:

Ability to stand, sit, or ambulate for long periods. Ability to mobilize freely around units, transport self to site locations, and perform required data entry into EMR and alternate data management systems.
