Cayuga Health System is a comprehensive healthcare network providing seamless, community-centered care for the residents of central New York.

We are seeking a skilled Inpatient Case Manager to join our team, responsible for delivering high-quality care to assigned patients.

This role involves assessing patient and care team needs, developing and implementing an appropriate Plan of Care, and making changes in response to changing patient needs.

The ideal candidate will guide the utilization review process, ensuring appropriate medical necessity, level of care, and patient status are met.

Key responsibilities include:

  • Evaluating hospital admissions using evidence-based standards of care
  • Communicating effectively with providers and care teams
  • Actively participating in multidisciplinary rounds
  • Issuing federal and state notices as indicated
  • Serving as discharge planner for patients
  • Assessing patient risk for readmission and identifying community support systems
  • Ensuring patient demographics, insurance, and primary care provider are up to date in EMR
  • Facilitating referrals to support teams and community-based organizations
  • Working to identify and eliminate barriers to discharge

Requirements include:

  • Graduation from a Masters of Social Work program
  • Current unrestricted NY State Licensure, LMSW
  • Minimum 1 to 3 years' experience in an acute care hospital setting or similar healthcare case experience
  • Knowledge of community-based organizations and strong communication, organizational, critical thinking, and problem-solving skills
  • Computer literacy and knowledge of EMR systems

We offer a competitive salary range of $34.50 to $47.50 and a dynamic work environment.
