Position Title:
LTSS Service Coordinator - Linn County

Job Overview:
This role is primarily field-based, requiring direct engagement with clients in the Linn County area. The LTSS Service Coordinator is tasked with overseeing service coordination for a specific group of individuals enrolled in specialized programs.

In partnership with the individuals served, the coordinator will facilitate the Person Centered Planning process, ensuring that the preferences, needs, and self-identified goals of each member are documented. This includes conducting assessments and developing a comprehensive Person Centered Support Plan (PCSP) along with a backup plan.

The coordinator will collaborate with Medical Directors and participate in interdisciplinary care rounds to support the creation of a fully integrated care plan.

Engagement with the member's support network is essential, as is the overall management of the individual's physical health (PH), behavioral health (BH), and long-term services and supports (LTSS) needs, in accordance with applicable laws and contractual obligations.


Key Responsibilities:

- Conduct face-to-face program assessments utilizing various tools and predefined questions to identify needs and apply motivational interviewing techniques for evaluations, coordination, and management of individual waiver needs (such as LTSS/IDD) and BH or PH requirements.

- Identify members with potential clinical healthcare needs, including risks for complications, and coordinate their cases with the clinical healthcare management and interdisciplinary team to provide effective care coordination.

- Manage the non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities to ensure efficient and cost-effective utilization of long-term services and supports.

- Document short and long-term service and support goals as directed by the member, collaborating with their chosen care team, which may include caregivers, family, natural supports, service providers, and physicians.

- Identify members who may benefit from alternative service levels or other waiver programs.

- Serve as a mentor or subject matter expert for new staff, assisting in training and participating in process improvement initiatives.

- Submit utilization and authorization requests to management, ensuring documentation aligns with the individual's care plan.

- Report critical incidents to the appropriate internal and external entities, including state and county agencies.

- Participate in appeals, grievances, and state audits as required.

- Travel to various locations as necessary.


Minimum Qualifications:

- A Bachelor’s degree and a minimum of two years of experience in a social work agency, or an equivalent combination of education and experience.

- Specific educational requirements and experience may vary based on state law and contract stipulations.


Preferred Qualifications:

- A Bachelor’s degree in a healthcare-related field is preferred.
